| The Prescription Drug Assistance Program (a.k.a. Compassionate Care) helps people who find it difficult to afford their expensive medicines receive FREE BRAND NAME MEDICINES from drug manufacturers, or generic medicines at a reduced price. You may qualify to receive
Free Medicines if:
1. You cannot pay for your medications,
or find it extremely difficult to buy them
2. You do not have any prescription drug
coverage
3. You meet the income guidelines
4. You are a U.S. Resident

| For more than eight contact our office. Guidelines are based on the Federal Poverty Level. Note: Financial guidelines may be greater or less than those specified above, due to the guidelines of the drug companies. Don’t hesitate to contact our office to verify eligibility. |
Last year our Staff helped patients receive over $1.2
million dollars in prescription drugs. We are excited
about the opportunity to help you by serving as
your Advocate.
We will fill out all the paperwork for you so that you
can get your medicines, either free or at a reduced
cost.
Your responsibility is to sign and date the
applications for the medicines and return them to
our office for further processing.
We will contact your health care professional to get
your prescriptions and have them sign and date the
applications. Once these steps are completed, we
will attached proof of income to the applications and
mail them to the companies who provide the
medicine. The medicine will either be delivered to
your home or to your doctor’s office for dispensing.
Most applications we submit are for 90 days of
medicine.
If you wish to participate in our program, contact
our office to find out, if the medicines you take are
available. We charge a $15.00 yearly fee to be in
the program, and $7.00 to complete each
application. These minimal fees help cover our
